A comprehensive look at the client’s childhood and family history, prior treatment, cognitive and behavioral symptomology, and goals.
Individual counseling is conducted with a trained therapist in a setting focused entirely on the client and their presenting needs in order to address underlying issues that may be contributing to maladaptive coping and behavior. The duration of Individual therapy varies on the need of the client and his/her response to the specific treatment methods used.
Family Counseling is conducted with the individual client and includes the involvement of family members and other close individuals. The purpose of this method of counseling is to provide the client with assistance in developing positive family interactions, improving the functioning of the family unit and developing supportive family involvement in the client’s treatment.
Group counseling is facilitated in a group setting with a collection of clients who have similar therapeutic needs. A working group of peers assist one another in opening up to address a variety of mental and behavioral health issues, give a forum for learning appropriate social interactions, and provide accountability amongst the members. Group counseling may focus on specific education, process, or a variety of topics associated with the needs of the group members.
Psychosocial Rehabilitation is a one to one service to teach and guide the client toward behavioral change and insight in areas such as behavior management, social competency, problem identification and resolution, effective communication, moral reasoning, identity and emotional intimacy, self-sufficiency, and life goals. The purpose is to teach skills that were never learned or restore skills that have been disrupted by trauma, injury, or illness and assist the client to reaching their highest level of functioning.
Basic Skills Training is a one to one service that focuses on age appropriate skills with which the client struggles. The purpose is to address and teach skills such as basic living and self-care, social skills, communication, parent training, organization and time management. The purpose is to teach skills that were never learned or restore skills that have been disrupted by trauma, injury, or illness and assist the client to reaching their highest level of functioning.
Whether in an individual or group setting, substance abuse focuses on the client who uses substances to cope with daily life. Clients are educated regarding the pattern of harmful use of any substance for mood-altering purposes. Clients are encouraged to explore the impact substance use has had physically, socially, psychologically, legally, and emotionally in their lives. Clients are also taught to explore the underlying issues leading to abuse or dependency and face the consequences that are directly related to ongoing use such as the removal of children, strained familial relationships, loss of employment, legal issues, and health concerns, etc. Relapse prevention is a significant part of this service.
Crisis intervention refers to the methods used to offer immediate, short-term help to individuals who experience an event that produces emotional, mental, physical, and behavioral distress or problems.
This program provides goal-directed, comprehensive and interdisciplinary services directed at attaining or maintaining individuals at the highest practicable level of physical, affective, behavioral and cognitive functioning. Mental and behavioral health staff is trained to work within nursing facilities to address behavioral issues due to trauma, mental health, or challenges brought on by, or exacerbated by health problems. Therapists conduct weekly sessions to address depression, anxiety, and a variety of other mental health diagnosis, while behavioral health staff focuses on basic skills training such as basic living and self-care, social skills, communication, organization and time management. The purpose is to teach skills that were never learned or restore skills that have been disrupted by trauma, injury, or illness and assist the client to reaching their highest level of functioning as they prepare for release and independent living.
A facility pay service where behavioral health services are provided to residents regardless of pay source to work on behaviors that are inhibiting transition to a lower level of care or long-term care facility.
A facility pay program where Legacy provides, education, support, coordination and submission of packets to DHCFP for Tier and then provides the behavioral health services to assist in a reduction of behaviors that challenge both facility staff and the health and welfare of the resident.
Legacy strives to provide clients with the best and most appropriate services. If a client has needs we are unable to meet or that require specialized services better provided elsewhere, Legacy will offer referrals to the client and encourage them to follow up with them. Legacy also provides clients with referrals and resources for basic needs and individualized assistance. We also provide referrals for programs to help our clients move toward greater self-sufficiency, including our personal development programs that teach skills such as financial planning, health and nutrition, organizational development and problem solving.
